Would you be able to provide your critical health care information to a new doctor, physical therapist or an emergency responder? The key to answering these questions accurately is creating and maintaining a personal health care record. This is why Fairfax County’s Long Term Care Coordinating Committee is hosting these free presentations on how to do this.
