The Arc of Colorado is collecting information about your experience with guardianship. The survey will remain open until February 15, 2024 and is designed to take less than 10 minutes. If you need assistance with the form please call 303-864-9794.
It will prompt you to answer a question about your reason for taking the survey. Are you a professional in the field or a friend of someone who has a disability? The form also asks if you or someone you care for has been recommended a guardian or conservator. If so, what is your experience.
There are 14 local chapters of The Arc in Colorado, and each advocates in the form of referral services, education, and public policy support for children and adults who live with intellectual and developmental issues.
