Family Caregiver Resources for Utah

No matter where you are in your caregiving journey — starting to plan; taking care of a family member in your home, in a facility, or from a distance; or managing end-of-life caregiving responsibilities — having resources at your fingertips will make the process easier.

AARP developed this family caregiver guide with you, the caregiver, in mind and as a starting point to help you find the services and support you might need throughout your journey.

Caregivers Support

Homecare & Hospice Association of Utah Family Caregiver Program (801-312-9110)
Offers family caregivers a free association membership, which includes information, advocacy, support and access to resources including caregiving tips, peer support and webinars.

Finances

Community Action Partnership of Utah (801-433-3025)
Provides services to low-income families and individuals.

Supplemental Nutrition Assistance Program (877-313-4717; 866-526-3663)
Eligible households receive food assistance through an Electronic Benefits Transfer card to cover a portion of their food budget, as well as nutritional education.

Utah HEAT Program (866-205-4357)
Provides financial assistance for residents unable to pay utility bills. Weatherization resources may also be available.

Legal

Adult Protective Services (801-538-3910 Salt Lake County; 800-371-7897 All other counties)
Provides education about and investigates allegations of elder abuse and neglect including physical, sexual, emotional, verbal and financial exploitation.

Catholic Community Services of Utah (801-363-7710)
Provides emergency assistance and legal advice.

State Courts Self-Help Center (888-683-0009)
Helps people understand their rights and responsibilities and provides information about addressing their legal problems if they cannot afford an attorney or chose not to hire one.

Utah Legal Services (801-328-8891; 800-662-4245)
Offers free or low-cost legal services for older adults and provides legal advice, information and assistance with noncriminal problems.

Health

Department of Health — Age Well (801-538-6003; 888-222-2542)
Provides information about diseases and conditions, medical screenings, applying for Medicaid and other services.

Department of Health – Division of Medicaid and Health Financing (800-662-9651)
Helps with medical costs for people with limited income and resources. Medicaid also offers benefits not normally covered by Medicare, including nursing home care and personal care services.

National Alliance on Mental Illness — Utah (801-323-9900; 877-230-6264)
Offers free support groups and education for those impacted by mental health conditions throughout Utah and their family members or loved ones.

Senior Health Insurance Information Program (801-538-3910 | 800-541-7735)
Provides one-on-one counseling and assistance to people with Medicare and their families. Helps obtain benefits, file claims and understand health care options.

Utah Alzheimer’s Association (801-265-1944; 800-272-3900)
Provides resources and support to anyone affected by any form of dementia including consultations, referrals, support groups, training and education along with a 24/7 helpline, online resources and advocacy.

    Housing and Care Facilities

    Long-Term Care Ombudsman (801-538-3924; 877-424-4640)
    Provides consumer protection advocacy services to residents of long-term care facilities such as nursing homes, assisted living facilities, specialty care facilities and boarding homes. Anyone can use an ombudsman to report a concern and get help resolving problems, including residents, residents’ family members and friends and long-term care facility employees.

    Utah Non-Profit Housing Corporation (801-364-6117)
    Provides affordable housing for low-income residents.

    Utah Housing Corporation (801-902-8200; 800-284-6950)
    Provides affordable housing for low- and moderate-income people.

    Information and Services

    Aging and Adult Services (877-424-4640)
    Provides free services to residents 60-plus such as home delivered meals, evidence-based health promotion, nutrition, education, in-home assistance, outreach, legal services, transportation, long-term care ombudsman, adult protective services and more.

    Services for People with Disabilities (801-538-4200; 844-275-3773)
    Manages resources for older adults and persons with disabilities and their caregivers.

    Utah Transit Authority (801-743-3882)
    Provides public transportation, paratransit/flex routes for eligible riders and reduced bus fares for seniors.

    Utah Division of Aging and Adult Services – Area Agencies on Aging (877-424-4640)
    Contracts with 12 Area Agencies on Aging to provide free services to residents 60 years and older. Services include caregiving support and respite care, congregate and home-delivered meals, evidence-based health promotion, nutrition, education, in-home assistance, outreach, legal services, transportation, long-term care ombudsman and adult protective services.

    Hospice for Utah (801-576-1455)
    Provides hospice patients and their families resources to help make better choices regarding serious illness and end-of-life care.
