This innovative solution links health and social service organizations across Wyoming to enhance overall well-being through real-time information sharing and coordinated care.
Through our partnership, AARP Wyoming is helping expand awareness of CommuniCare because we believe in making it easier for older adults, caregivers, and families to access the help they need. Partnering with 211 strengthens our shared mission: supporting Wyomingites through every stage of aging by connecting them to trusted local services and support systems.
CommuniCare, CIE empowers organizations to work collaboratively, ensuring that no individual falls through the cracks. By streamlining referrals and connecting care team members with critical client data, the CIE helps agencies deliver more effective, person-centered services.
