The Wyoming Department of Health, Division of Healthcare Financing is hosting stakeholder forums in communities throughout the state to discuss upcoming changes to the Community Choices Waiver (CCW).
The Community Choices Waiver (CCW) targets adults 19-64 with a physical disability and aged adults 65+. The level of care needed to be eligible for this waiver is Nursing Facility level of care. The CCW is provided as an alternative to institutional care and individuals must be financially eligible.
Some of the available services through the waiver include
- Case management
- Personal Care
- Respite Needs
- Home Delivered Meals
- Personal Emergency Response Systems
- Non-Medical Transportation
- Adult Daycare
- And more….
AARP nationwide is a strong supporter of Home and Community Based Services and encourages our members who are CCW users to attend these meetings. If you are not sure whether you do receive CCW funding, ask your in-home care provider.
The Department of Health invites CCW participants, providers, case managers, and other interested stakeholders to provide advice and feedback on the program.
